We register ANY living will/advance directive, including the popular Five Wishes (available for purchase here) in the United States of America. Your $25 registration fee includes the first year of storage in our secure database, and a durable plastic wallet card to identify you to hospitals and physicians. Pay a low $10 per year renewal fee to keep your document available to those who need access to it.
