How to Register an AHD

The MyHealthDirective.com system registers and stores Five Wishes and other Advance Healthcare Directives (AHD) for individuals residing in the United States of America.

To register, simply:

• Purchase your Five Wishes plus storage AHD from one of our Sponsors. If you have an existing AHD, click on the "Download Form" button to the upper left to download a Registration Agreement.
• Fill out the Five Wishes AHD form and have it witnessed or notarized.
• Submit your orginal Five Wishes AHD (witnessed or notarized) to MyHealthDirective.com. The Registration Agreement is inserted in the center of the document. If you are submitting an existing AHD, send the original with the Registration Agreement and payment to MyHealthDirective.com.

We will scan your AHD into our database, exactly as submitted. The original AHD will be filed and stored in a secure location.

After being entered into our system, you will receive a letter and wallet card that confirms you as a member of MyHealthDirective.com and provides you with a User ID and password. Your User ID and password will allow you and your healthcare professionals to enter the secured section of MyHealthDirective.com.

Remember, completing an AHD and registering it with MyHealthDirective.com is not a substitute for consulting with your own legal advisor and personal physician. Please consult with your legal advisor and physician regarding these matters - there may be a number of important issues in addition to your AHD that they will want to discuss with you.
